“Back to the Home” programme creates new responses for patients with discharge

A Law No. º 43/2026, published on 10 August, approves the legal framework for the ‘Back to Home’ programme, aimed at ensuring social and continuity of care responses for patients who, after discharge from the clinic, need support and do not have an adequate housing or family response.

The new regime privileges home-based and community-based solutions, promoting continuity of care, return to the usual way of life and the reduction of reinternations and situations of unnecessary institutionalization.

Among the main measures is the creation of the Transitional Residence, a temporary social response aimed at users in need of transitional care while an adequate and lasting solution is found.

The law also provides for new response modalities, financing mechanisms and monitoring of the program, as well as support for the adaptation of spaces and acquisition of equipment to strengthen the supply of vacancies in the solidarity network.

This law will take effect with the entry into force of the State Budget of 2027.
